Published for Joint Commission. The intent of the requirement is to reliably identify the individual as the person for whom the service or treatment is intended and to match the service or treatment to that individual.

A literature review of relevant peer-reviewed and grey literature published from January 2015 to October 2019 was conducted to inform the paper. The focus was on: 1) patient identification techniques and 2) unintended consequences and ramifications of unresolved patient identification issues.

Patient matching is defined as the identification and linking of one patient's data within and across health systems in order to obtain a comprehensive view of that patient's health care record. At a minimum, this is accomplished by linking multiple demographic data fields such as name, birth date, phone number, and address. Patient matching is a critical component to interoperability and the nation's health information technology infrastructure.

Procedural failures during medication administration include a lack of appropriate hand hygiene and failure to verify patient identification prior to administration of medication. Procedural failures are common among nurses and nursing students and can lead to serious patient harm.

Clinical Preceptorship Learning Model is a learning model that is implemented in hospitals with guidance supported by guidance, assistance for nursing students, who are currently educating in hospitals. A clinical Preceptorship implementation model that will determine learning competence and also patient safety planning.

Objective: to identify the perception of nurses who work in a pediatric unit about patient safety measures. Method: qualitative, descriptive and exploratory study, conducted in a public hospital in Pernambuco, Brazil. Six pediatric nurses participated. Data collection was conducted between November and February 2021 through semi-structured interviews; data analysis was conducted through content investigation. Results: three categories were identified from the analysis of the results: nurses' perception of patient safety, patient identification barriers in the pediatric unit, and identification wristbands: a strategy to mitigate errors in the pediatric unit. Final Considerations: it was possible to realize that nurses believe in the importance of patient identification as a strategy to minimize the occurrence of medication-related errors and emphasize that this act provides greater safety to professionals during the execution of procedures.

Objective: to indicate the forms of identification of the patients used in the period of hospitalization in an infirmary, and how their maintenance is guaranteed, in the search for improvements in daily practice, and reduction of adverse events related to patient identification.
